In this workshop learn best practice principles on implementing the person's plan to enable an efficient and collaborative discharge. This workshop is designed for hospital clinicians and support coordinators who have had basic exposure to the health and NDIS interface.

After this workshop you will be able to:

  • Assist an NDIS participant to implement their plan to facilitate discharge
  • Determine when a change in circumstance is required and how to avoid discharge delays
  • Describe the roles and responsibilities of hospital clinicians and support coordinators who are facilitating discharge
  • Coordinate and collaborate with stakeholders to facilitate a timely discharge from hospital
  • Problem solve complex scenarios and implement strategies to avoid discharge delays
