There is confusion among hospital teams regarding hospital discharge processes for NDIS participants, particularly relating to use of templates, involvement of community occupational therapists (OTs) in hospitals, the role of hospital liaison officers (HLOs) and implementation of the hospital discharge operational plan.

Hear from the NDIA about:

  • The implementation and progress of the hospital operational plan  
  • When to use NDIA templates and who should complete them
  • Escalation routes, including the role for HLOs in reviews and appeals
  • Responsibilities and approved delegations of HLOs and specialist discharge planners
  • When to use NDIS plan funding to support participants discharging from hospital (including community OTs and training support workers)

About the presenters

Facilitated by Whitney Lipman, UpSkill Trainer

Whitney delivers capacity building training on the NDIS and hospital interface. Before joining the Summer Foundation, Whitney worked in the public health setting for more than 10 years as a senior occupational therapist working to facilitate effective discharges across the continuum. Whitney has a Masters in Clinical Rehabilitation and has also worked as a NDIS project officer for Monash Health.
